The mahi of management in Bulls: Pressure is the name of the game in rural general practice

Karen Greer describes a day in the life of a medium-sized rural practice

In common with many other general practices, the Bulls Medical Centre team has been working extremely hard and for long hours in the past few years as a result of COVID-19 and other increasing demands.

With secondary care under ongoing pressures, patient care is transferring back to primary healthcare due to lack of capacity. This means that, increasingly, complex patients are returning to our practices for appointments, putting high pressure on our team and making life difficult.

Because of our location at Bulls on State Highway 1, we also have casual patients drop in while driving through, as well as call-outs to emergencies. This is part of the fun of rural practice but it is also a challenge.

Like many practices, we are short-staffed. Limited locums and casual staff mean our team cannot go on leave, and their leave is accumulating.

Our pool of casual nurses has left and gone overseas. We have been advertising for nurses for quite some time now and have had to develop a health care assistant role to help with the pressure on our team. Health care assistant is an exciting new role.

Ongoing pay negotiations are a concern. We passionately support pay equality for primary care nurses but no resolution had emerged at the time of writing.

We find recruitment and retention of receptionists and administrators is a problem as well.

Audits to be completed, feedback, complaints, system changes, Foundation Standard, Cornerstone – these are all part of the complex demands of the practice manager’s role.

I rely on my colleagues in the Practice Managers and Administrators Association of New Zealand. This is a fantastic organisation with a website, Facebook page and Messenger. It is amazing how we can all make comments with our roles, and everything can be so different, but as a team we are all on the same page. Keeping our team and patients happy.

I am sure many practices face these challenges. For all those in rural practice who feel overworked and undervalued and who are working extremely hard, we really appreciate all the mahi you put in.
